Magento is NOT working, Template Paths Turned off, yet one still appearing?

I just did a fresh install of magento, and I am using the theme heirarchy to edit the layout of my site, leaving default as it is and using my_theme to display any updated files. However somewhere in the proccess something has gone wrong.

I switched on the template path hints in the admin, then I switched them off, however the hint around my navigation bar is still there and will not go away? Does anyone know what could have caused this.

Also I have noticed that no changes to the XML files work. For example I tried to edit the catalog.xml file to move the navigation bar down to the left hand column, by changing the reference name=”left” instead of header. Which is how I edited it in my last installation, and then uploaded it to default/my_theme/layout/catalog.xml … But there was no changes, I also tried replacing the default catalog.xml with the updated file which also made no change?
